Questions

What comes with the response form?

The CP2000 includes a Response form, an envelope and a list of payment methods.

The first page sums up the proposed changes. It gives a phone number and the steps to respond.

What should I check before I mark anything?

The notice lists the amounts you reported and the amounts each payer reported.

It also shows each payer’s name and ID number, and the type of form, such as a W-2 or 1099.

Compare each item with your return and your own records.

How do I fill it out?

Here are the steps the IRS describes, in order.

Mark agree or disagree Indicate on the form whether you agree or disagree with the proposed changes.

Authorize someone, if you want To let someone else contact the IRS about the notice, fill in the Authorization section.

Sign and date Complete and sign the form. Both spouses sign if you filed married filing jointly.

Attach your statement and copies If you disagree, add a signed statement saying why, with copies of your documents.

Send it by the response date Upload it, or fax or mail it to the number or address on the notice.

Follow the instructions printed on your own form. They come first.

What if I agree and want to pay?

You can pay online now, or wait for the IRS to adjust your account and send a bill.

Paying, or applying for a payment plan, doesn’t replace the form. You still sign and return it by the due date.

What if I lost the envelope?

Send your reply to the address on the first page of the Response form.

Or upload it, or fax it to the number at the top of the notice.

No response form at all? Follow the instructions on the notice itself.
